Submitting a Claim by Email
In order to submit a claim by email please follow the instructions below:
- Click the appropriate link to open the form (here)
- Fill out the form as completely as possible

Once the form has been completed there are two options for submitting a claim by email.
You may either manually save the form and attach it to an email (here), or use the automatic
submission feature built into the form using the "Submit Form" button (here).
Manually saving and emailing a claims form
In order to save the form, you may either click the save icon
Or click "File" -> "Save As" in the toolbar
Save the form to a place of your choosing, then open your email client and attach the form to
an email addressed to Claims@MidwestClaims.com, with the subject "Submitting Completed Form."
Using the "Submit Form" button to email a claims form
Using the "Submit Form" button will automatically attach the form to an email addressed to us.
This option works best if you are using a desktop email application such as Microsoft Outlook.
However, if you are using an internet browser-based email application such as Gmail, Hotmail,
or Yahoo! the "Submit Form" button may not work as expected.

Click the "Submit Form" button

In the "Send Form" menu, enter your name and email address

Next choose how you would like to email the form.

If you are using a desktop email client such as Microsoft Outlook,
select "Desktop Email Application."
This option will automatically attach the form to an email addressed to us.

If you are using an internet browser-based email system,
please select "Internet Email."
This option will prompt you to save the form to
your computer so you can manually attach it to an email later.

If you chose "Desktop Email Application" you may receive the following message:
If you do see this message please click "Yes" to continue

Once complete, the form will automatically be attached to an email and sent to us.
There is no need to enter our email address, the form will do it for you.
Once you have completed these steps and the form has been submitted,
you may want to check your email client's Outbox and/or Sent Items folder
to verify that the message was sent.
